The mssql problem is fixed, patch uploaded to wiki.

Actually I never found the exact cause. I just took advantage of the 
fact that mssql and mssql05 are exactly the same except for which hash 
they pick, and the fact mssql upper-cases the keys.

OMP support for mssql was removed in the process. It didn't scale well 
anyway for such a fast format.

The code is now very *very* similar between these formats so any changes 
to one will probably apply to the other.
