Subject: external mode VM speedup

Hi,

I committed some changes to the external mode virtual machine, which
speed it up a little bit - in my testing, by 5% to 10% for AppendLuhn,
Keyboard, Strip.  Already-fast external modes don't benefit from this
much or at all; this includes KnownForce and especially DumbForce.

In case anyone wants to help test this (before it becomes part of the
next release), use this patch and provide your feedback:

There's more room for improvement, but I did not want to go for invasive
changes yet.  I think we have other priorities now, and this was already
a little bit of a distraction (sorry!)
